Tenesmus is a sensation of incomplete evacuation of the rectum following defaecation, or the desire to defaecate when the rectum is empty. It may be a complication of ulcerative colitis when the rectum is involved, or be due to the presence of a tumour in the rectum. Most usually, however, it is due to irritable bowel syndrome.
